Becoming A New Patient

We are accepting new patients.

Please note that you need to submit paperwork prior to scheduling your appointment as Dr. Hammes reviews all the requests, assigns the provider and the length of time required for the visit.

If you choose to fill out the comprehensive new patient form online, be aware that completing the form may take 30-60 minutes to complete. If you prefer to have more time, please print the PDF and email us a copy when you are done.

Every new patient is required to submit a $50.00 deposit that will reserve your appointment time and your deposit will be applied to your first appointment’s bill. If you decide to cancel within 24 hours of your appointment or you do not come to your appointment, you will not be refunded your deposit.

Integrative Wellness is a fee for service practice which means payment is expected at your time of service. We accept all major credits cards, health savings accounts, checks and cash for payment. We can provide a Superbill (HCFA) form to those that have out of network health insurance coverage who would like to submit.

Our practice is opted out of Medicare.
